CHILDREN'S SPECIAL HEALTH CARE SERVICES


Financial Assistance is provided for medical care for families with children having certain disabilities or chronic illness. Families of any income can qualify for this program. Care coordination and case management are provided by a Public Health Nurse. The staff can also provide Medical Outreach and Advocacy during the Healthy Kids (Medicaid ) /MIChild Insurance Application process for potential recipients, as well as assistance to eligible children in the completion of a shortened insurance form.

ELIGIBILITY


Funds are available to support preventative efforts and to provide assistance, transportation, and lodging for families of children (under 21) with certain disabilities or chronic illnesses including:

  • AIDS and HIV infection
  • Congenital birth defects
  • Hearing impairments and chronic ear infections
  • Cataracts or crossed eyes
  • Sickle cell disease
  • Apnea (breathing problems at birth)
  • Some seizures disorders
  • Severe asthma
  • Some allergies
  • Diabetes
  • Cancer
  • Orthopedic conditions
  • Cardiac problems
  • Hemophilia (lifetime coverage)
  • Cystic fibrosis (lifetime coverage)

(You must be under the care of a CSHCS participating physician or be referred to one by your attending physician. If you cannot pay for a CSHCS participating physician, help may be available)
