FIMR uses a community-based approach by bringing together local health providers, consumers, advocates and leaders. Jackson County FIMR is modeled after the national FIMR program, which is two-tiered, consisting of a Case Review Team and a Community Action Team.
The Case Review Team is composed of human services agencies, health care providers and other experts that review case information and interviews. They identify issues, look at trends, and make recommendations to the Community Action Team for community change.
The Jackson County Maternal Child Health Action Team has identified three major goals for improvement:
